LEARN ABOUT HOW TO DISCOVER YOUR HERITAGE
Illinois Adoptees born and adopted after Jan. 1, 1946 are now able to acquire a copy of their original
birth certificate. This law went into effect in November 2011. If you were born before that date,
your records were never sealed and you could have requested your original birth certificate
at any time.
Please print and fill out the form at the site below, being sure to include a copy of your non
expired drivers license or any government-issued photo ID along with a check for $15 made
payable to IDPH and mail to the address at the top of the form. Currently, it is taking about three
weeks to receive it.

(form to be filled out by the adoptee or of a family member of a deceased adoptee)
Once you have obtained your original birth certificate, please notify us with your mother's name,
her age at the time of your birth, state where she was born as well as the address that was provided
as her place of residence, so we can update your registration and file. You can simply send an
